Completing the Centre Declaration Form at the end of the test

When a test has been completed, you must submit the ‘End of Test Declaration’ form. 


The ‘End of Test Declaration’ form is available in the Invigilation Centre Portal once the test has been completed


Select the ‘Assessments’ tab in the upper left hand corner.

 


Any completed tests will be highlighted orange. 


Select the button 'SUBMIT END OF TEST DECLARATION' to complete the declarations.

You will do this for each of the four tests.




Please ensure you have read the Invigilation Centre Guide for information on malpractice before completing the declaration.


I DON'T HAVE ANYTHING TO REPORT


If you have nothing to report, select the checkbox at the top of the list to bulk-select all the names. You may need to move through the pages to select everyone on each page.


IMPORTANT: This information is visible to the senior schools applied to and the guardians.


If there were no incidents of malpractice to report, you should select No.

You will then be prompted for the invigilator's name and the name of the person completing the declaration.  This might be the same person.


Do not tick the 'I wish to report important information ...' box.

Select submit to complete.


I HAVE SOMETHING TO REPORT


You can create a Declaration Form for:

1. An individual applicant

2. A group of applicants

3. The whole cohort 


1. COMPLETING FOR AN INDIVIDUAL APPLICANT

If there was an issue for a specific applicant (such as medical reasons, technical issues or for their SEND adjustments), then you should add this declaration first.

Select the applicant you wish to create a declaration for by using the checkbox on the left-hand side. 


IMPORTANT: This information is visible to the senior schools applied to and the guardians.


If there were no incidents of malpractice to report, you should select No.

You will then be prompted for the invigilator's name and the name of the person completing the declaration.  This might be the same person.


To add specific information relating to this applicant, tick the box to open the free text box.

Do not write the names of any other pupils in this section.

Select submit to complete.

Each individual declaration you wish to make should be done separately by selecting the checkbox next to the pupil's name.


Once you have completed all your individual declarations you can then complete the rest of the cohort in one go (see COMPLETING FOR THE WHOLE COHORT).

2. COMPLETING FOR A GROUP OF APPLICANTS

If there was an issue for a group of applicants (such as technical issues or a disturbance such as a fire alarm), then you should add these declarations next.

Select the applicants you wish to create a declaration for by using the checkbox on the left-hand side. 


IMPORTANT: This information is visible to the senior schools applied to and the guardians.


If there were no incidents of malpractice to report, you should select No.

You will then be prompted for the invigilator's name and the name of the person completing the declaration.  This might be the same person.


To add specific information relating to this group, tick the box to open the free text box.

Select submit to complete.

Do not write the names of any other pupils in this section.  Anything written in this section is shared with the guardians of all the applicants that are ticked.



3. COMPLETING FOR THE WHOLE COHORT

If there was an issue for the whole cohort (such as technical issues or a disturbance such as a fire alarm), select the checkbox at the top of the list to bulk select the names. You may need to move through the pages to select everyone on each page.


IMPORTANT: This information is visible to the senior schools applied to and the guardians.


If there were no incidents of malpractice to report, you should select No.

You will then be prompted for the invigilator's name and the name of the person completing the declaration.  This might be the same person.


To add specific information relating to the whole cohort, tick the box to open the free text box.


Do not write the names of any other pupils in this section. Anything written in this section is shared with the guardians of the whole cohort.
